Mary Vinson Memorial Library Meeting Room Request

In order to request use of the Meeting Room the Twin Lakes Library System requests that you begin the process by filling out this simple online form. Upon the completion of this form Library Administration will contact you within three business days in order to finalize details. Please be sure to have read the Meeting Room Policy.

Must be 18 or older and must also be in attendance at the meeting in order to reserve the room.
PINES Library Card Number: *

Please enter your library card number without spaces. This is a 14-digit number. You must have a valid PINES Library Card in order to reserve the room. In addition, your card account must be in good standing as any damage caused by the user's meeting will result in charges being placed on the user's account.
Date of Meeting: * / MM / DD YYYY
Subject to availability and is on a first-come, first-serve basis and may be reserved up to one month in advance of the scheduled date. Due to unforeseen events, the Library reserves the right to cancel any pre-scheduled meeting. This will not be done lightly but it is inevitable that complications will arise from time to time.
Meeting Start Time: * : HH : MM SS AM/PM
There are presently no time restrictions or limitations on the use of the room. However, the meeting room will close twenty minutes prior to the closing of the Library facility.
Meeting End Time: * : HH : MM SS AM/PM
There are presently no time restrictions or limitations on the use of the room. However, as others will also need to schedule to use the room, please approximate when you will be finished using the room.
Organization/Group Name: *

Please include the name of the organization/group that will be using the room. For additional information on "who may not use the library meeting room" please refer to the Meeting Room Policy.

Please note that the number of attendees shall be limited to the maximum capacity as determined by the Fire Department. Seating is available to no more than twelve (12) individuals.
